Medical Reimbursement Claims
  1. Completed Medical Reimbursement Form
  2. Official Receipts (Original)
  3. Medical Certificate to include final diagnosis (For Inpatient and Outpatient Laboratory)
  4. Medical Abstract (If applicable)
  5. Doctor’s order for laboratory tests/procedures
  6. Doctor’s prescription for outpatient medicines
  7. Itemized Statement of Account (inpatient/confinement)
  8. Police report (vehicular accident)/Affidavit of Accident (for other incidents)
Motor Car | Motorcycle Claim

​​Own Damage and Acts of Nature Claim

  1. Filled out claim form
  2. Police report or a notarized affidavit about the accident
  3. Photocopy of the Certificate of Registration with the Official Receipt
  4. Photocopy of the driver´s license with the Official Receipt
  5. Pictures of the damaged portion(s) of the insured unit, with at least one showing the plate number
  6. Repair estimate
  7. Policy Number (ensure that the policy is paid)

 

 Third Party Liability – Property Damage Claim

  1. Filled out claim form 
  2. Police report 
  3. Photocopy of the Certificate of Registration with the Official Receipt (both parties)
  4. Photocopy of the driver´s license with the Official Receipt (both parties)
  5. Pictures of the other vehicle (or property) showing the car’s plate number and the damaged portion(s)
  6. Repair estimate
  7. Policy Number (ensure that the policy is paid)

 

Third Party Liability – Bodily Injury Claim

  1. Filled out claim form
  2. Police Report
  3. Original copies of the official receipt covering hospital bills, medicines and/or supplies
  4. Certified true copy of birth certificate is required if the victim is a minor or proof of relationship of claimant to the victim
  5. Pictures related to the accident or damages
  6. Photocopy of the Certificate of Registration with the Official Receipt
  7. Photocopy of the driver´s license with the Official Receipt
  8. Original copy of the medical certificate, death certificate, or autopsy report (if applicable)
  9. Policy Number (ensure that the policy is paid)
Property | Fire Insurance Claim
  1. Filled out claim form
  2. Policy Number
  3. Pictures of the damaged properties
  4. A detailed inventory of the damaged insured property, including valuation for repair or replacement and an estimated salvage value.
  5. Please provide an incident report outlining the circumstances surrounding the loss, including any available witness names, the location and ownership details of the insured property, and the year of construction along with the original construction or building cost.
  6. Arson Investigation Report or Fire Certification
  7. PAG-ASA Report (for lightning, typhoon, and other weather disturbance losses)
  8. Other documents that may be required by the adjuster upon inspection
Travel Insurance Claims

Basic Claim Requirements

  1. Filled out claim form
  2. Policy Number
  3. Photocopy of passport with pages showing dates of departure and arrival corresponding with the itinerary on the policy contract
  4. Flight itinerary, both original and revised flight details if applicable

Baggage Delay Benefit

  2. Original Acknowledgement Receipt / Form stating the exact date and time baggage was retrieved

 

Strikes and Hijack Benefit

Hospital Income Benefit

  • Itemized/Detailed Statement of Account
  • Discharge Summary / Medical Abstract

 

Funeral and Burial Expense

  • Death Certificate
  • Original Official Receipts of Funeral and Burial Expenses

 

Personal Liability benefit

  • Original Official Receipts incurred for such liability

Loss of Travel Documents Benefit

  • Original Itinerary
  • Copy of the replacement passport / Travel Tickets

  • Original Receipts on the expenses incurred for the replacement of passport / Travel Tickets

 

Trip Cancellation

  • Proof of advance payment made for travel and accommodation expenses
  • Penalties and other irrecoverable pre-paid charges related to the trip
  • Legal documents proving trip cancellation with specified non-refundable portion
  • Copy of the following documents:
    • Death Certificate, in case of death by insured or direct members of the family
    • Full doctor’s report on the emergency of insure or direct members of the family 
    • Original physician’s declaration of patient unfit to travel
    • In case of medical treatment or death of insured’s immediate family: proof of relationship with the immediate family member (birth certificate, marriage certificate, etc.)
    • Public documents I.e. newspaper, news portal, etc. or official advisory that report: natural catastrophes, outbreak of strike or riot, or acts of terrorism
    • Original irregularity report issued by airline / carrier / airport stating reason of cancellation

 

Flight Delay

  • Original irregularity report issued by airline/ carrier / airport stating reason of delay
  • Boarding pass
  • Original Official Receipts for expenses during delay

 

Missed Connecting Flight

  • Original irregularity report issued by airline/ carrier / airport stating reason of cancellation
  • Boarding passes (Actual and revised)
Marine Cargo Claim
  • Copy of the insurance policy
  • Formal advice from the insured to the insurer of the claim
  • The following documents have to be submitted:
  1. Commercial Invoice
  2. Bill of lading
  3. Packing list
  4. Copies of the formal claim against the following:
    1. Arrastre Operator / Contractor
    2. Vessel’s owner / Agent
    3. Therbailees, such as the customer broker, etc and their representative replies
  5. Copy of delivery receipts
  6. For claims involving shipments in Bad Order condition:
  1. Bad order issued by arrastre contractor
  2. Copy of the turn-over survey of the bad order on board cargoes issued by arrastre contractor as attested by the vessel’s representative
  3. Original labarotary or quality control analysis report
  4. Original invoice for repair / reconditioning of the bad order or damaged items
  1. For claims involving short-landed shipment / spillage / leakage 
  1. Bad order certificate issued by the arrastre contractor
  2. Turn-over survey on bad order cargoes
  1. For claims involving hort delivered shipment: short delivery or short landing certificate issued by the arrastre contractor certifying the non-discharge of packages or tally sheet of discharge
